Methylphenidate

Methylphenidate

(also known by the trade names Concerta, Ritalin)

Methylphenidate is a central nervous system stimulant and a medicine used for medical conditions, such as attention deficit hyperactivity disorder (ADHD) and narcolepsy [1]. In recreational doping, it is used for the perceived effect of enhancing cognition.

Mechanism of action

Methylphenidate accelerates the function of certain neurotransmitters in the brain [2]. More specifically, it works by blocking the reuptake of dopamine and norepinephrine in presynaptic neurons, thereby raising their levels in the synaptic gap [1].

Methylphenidate in cognitive-enhancing drug use – why is it used?

Methylphenidate is reported to be used by healthy individuals seeking to sharpen focus, memory, and productivity, a practice also referred to as “cognitive enhancement”. However, evidence of real cognitive benefit is inconsistent [1].

Research on the phenomenon:

  • Edinoff et al. (2022) review reports that among college students who used stimulants for nonmedical reasons, ‘academic enhancement’ was the most common reason (50-89%). [3]

Adverse effects*

Methylphenidate’s commonly recognized severe adverse effects are irregular heartbeats (heart palpitations), mood swings or fluctuations, or personality changes [2]. Other very common adverse effects include headache, nervousness, insomnia [1,2] and common adverse effects include the following symptoms [2]:

  • cardiovascular: high blood pressure, rapid heartbeat
  • neurological: vertigo, feeling of weakness, uncontrolled movements, hyperactivity
  • psychological: aggressiveness, agitation, tension, depression
  • gastrointestinal: loss of appetite or decreased appetite, digestive disturbances or stomach problems, stomach pain, diarrhea, nausea
  • musculoskeletal: joint pain, muscle cramps
  • sexual: decreased libido
  • respiratory: cough, sore throat, upper respiratory tract infection
  • dermatological: unusual hair loss or thinning, itching, rash

Additionally, higher doses are known to contribute to addiction mechanisms and increase the risk for dependence [1].

Medical use

Methylphenidate’s primary medical use is in attention deficit hyperactivity disorder (ADHD) and as a second-line therapy for narcolepsy [1]. Off-label use includes cancer-related fatigue and depression in older adults.
